Indications of Alkalina 600mg Tablet

Alkalina 600mg Tablet is an antacid medicine containing sodium bicarbonate as its active ingredient. It is used to treat heartburn and acid indigestion.

Pharmacology

Sodium bicarbonate, the active ingredient in Alkalina, acts as a systemic alkalizer. It increases plasma bicarbonate levels, buffers excess hydrogen ions, and elevates blood pH, thereby reversing the clinical manifestations of acidosis. Additionally, it serves as a urinary alkalizer, increasing the excretion of free bicarbonate ions in urine, and effectively raising urinary pH. Alkalina also helps replenish electrolyte imbalance in severe diarrhea, where bicarbonate loss can be significant.

Dosage & Administration of Alkalina 600mg Tablet

• Oral: The usual dose ranges from 300 mg to 2 grams, taken 1 to 4 times daily.
• Intravenous (for severe metabolic acidosis):
  • Adults: Administer slowly by injection of a hypertonic solution of up to 8.4% (1000 mmol/L) or by continuous infusion of a weaker solution, usually 1.26% (150 mmol/L). During advanced cardiac life support procedures, 50 ml of an 8.4% solution may be administered.
  • Elderly: Dosage adjustments may be necessary.
• In case of alkalosis, discontinue Alkalina and manage according to the severity of alkalosis present. Intravenous 0.9% sodium chloride injection may be given, and potassium chloride may be indicated if hypokalemia is present. Severe alkalosis symptoms like hyperirritability or tetany may be controlled with calcium gluconate. Ammonium chloride may be indicated in severe alkalosis.

Interaction

Alkalina may interact with several drugs, including aspirin, memantine, medications with enteric coating, corticosteroids, corticotropin, alkalis, calcium, diuretics, and potassium. It may also decrease the effectiveness of certain drugs such as ampicillin, atazanavir, ketoconazole, itraconazole, iron supplements, and sucralfate.

Contraindications

Alkalina is contraindicated in individuals with hypersensitivity, hypoventilation states, excessive chloride depletion, metabolic and/or respiratory alkalosis, hypocalcemia, and diuretics known to produce hypochloremic alkalosis.

Pregnancy & Lactation

Use during pregnancy is only recommended if the potential benefit justifies the potential risk to the fetus. Based on animal data showing adverse renal effects, Alkalina is not recommended during the second and third trimesters of pregnancy. It may also worsen high blood pressure. This medication may pass into breast milk.
